Job Description

Shift: Days

Status: 40 hours per week

GENERAL SUMMARY:

Reporting to the clinical manager or designee, coordinates specialty and other related clinical programs for Home Health Care. Utilizes the knowledge and application of midlevel provider or nursing process and principles to coordinate the provision of high quality, cost-effective care. Acts as consultant for home health care nursing staff and provides direct care to patients in the home.
 

Qualifications

EDUCATION/EXPERIENCE REQUIRED:

  • Degree in area of specialty: ASN; BSN, OT, PT, or MSW.
  • A minimum of three (3) years of clinical experience. Experience with hi-tech patient care. Experience in public/home health preferred.
  • Nurse Practitioner or certification in a Clinical Specialty Program preferred.
  • Progressive experience in program development and coordination desirable.
  • Interpersonal skills required to provide guidance and consultation to staff and external agencies.
  • Must possess a current State of Michigan driver's license and a satisfactory driving record.

CERTIFICATIONS/LICENSURES REQUIRED:

Current licensure in the State of Michigan as a Registered Nurse, Occupational Therapist, Physical Therapist, or Social Worker.
