Company Description

We are an outpatient medical group with offices throughout Central and South Florida.

Job Description

This company provides a turn-key operation wherein the physician fulfills the physician-required elements of companies weight loss program pursuant to the processes, which have been set in place over the past 25 years. The processes include the physician-patient component and ordering, inventorying, prescribing and dispensing the medications utilized by the company. See below.

The physician meets with each new patient and performs a chart review. This is performed by the physician after the history, and vitals have been taken and recorded, as part of the patient’s chart creation, by the staff. A blood sample is also extracted from the patient by the staff, which is subsequently reviewed by the physician for any contraindications

The physician’s patient approval is focused on the patient's medical capacity to participate in all or a portion of the weight loss program. While the staff is highly trained and experienced on the requirements and medical issues that may deem someone not medically approved for certain elements of the Program, a physician-conducted review is still necessary and required for approval. The physician typically sees 12-18 new patients a week, spread out over 5 day, M-F schedule, in addition to returning weekly patient’s, where necessary.

This company has a complete ordering, inventory, storing and dispensing system in place. The inventorying, prescribing and dispensing is done under the physician’s DEA and dispensing license and review. If the physician does not already have his/her dispensing license, the company assists the physician in this simple registration process.

The companys' clinical space and staff are operated separately from the physician's, including all staff hiring, training, management, patient record keeping, patient transactions, clinic operations in addition to the location build-out. The space ranges between 1700 to 2000 sf, consisting of a waiting room, reception area, medication room, consultation room, 2-3 weigh-in rooms, a supply room, break room, and consultation room.

For regulatory and ease-of-access purposes, the company generally requires the physician’s space/practice to be contiguous with the company's clinical space via a connecting doorway.

The physician does not design, build, or create any of the above systems or build-out, nor provide any of the equipment, software, furniture, et cetera. It is all provided. The physician separately runs their own clinic, their own staff, and their own space.

Typically, the physician’s time ranges between 2 and 4 hours a week in fulfilling the duties above. New patient visits last 1.5 to 2 hours. The physician has the freedom to see the patient anytime during the scheduled visit, providing the physician flexibility on scheduling and caring for their own practice’s patient and workflow. The company's scheduling calendar is provided to the physician and his staff. Transformations’ hours of operation are generally 3 days a week from 7am to 4pm, one day a week from 7am to 7pm, and on Friday, from 7am to 12 noon.  They are closed on weekends.

The physician is paid a set amount each month under an independent consultant's agreement, which may increase, depending on the volume of patients over time. The company also often generates substantial traffic flow and new business exposure for the physician--the physician is examining 12-18 new patients a week (many that may not have a primary doctor) in addition to the weekly repeat patient traffic flow.

The weight loss program is broken down into 2 primary stages, whereby the patient goes through a series of adjustments in the medications and dietary standards per the protocol of the Program, and their progress. It is a dietary-based program, so each patient's time on the medications is limited.  After completion of the 2 stages, they then recycle back through the stages until they meet their target weight.
