Job Description
Summary of Duties:
- The LPN performs delegated nursing and medical functions of data collection and delivery of care to an assigned group of patients, or individualized patient. The LPN works under the direction of a Registered Nurse, or physician/authorized practitioner and in collaboration with other members of the healthcare team. The LPN is responsible for adhering to all policies, procedures, and nursing practice guidelines.
- Collects data for patient assessments.
- Delivers care as delegated by the RN or physician/authorized practitioner to assigned patient population, or individual patient.
- Communicates effectively with other members of the healthcare team.
- Demonstrates ability to provide care or service adjusting approaches to reflect developmental level of population served. (as applicable)
- Performs other duties including Performance Goals developed by manager and employee and reported in the employee’s Performance Review as part of UMPhysicians’ Performance Management tools
Monday-Friday, Daytime shift
Pay rate starting at $17/hr (based on experience)

Qualifications
Requirements:
- Graduate from an accredited LPN program - Required
- Current MN LPN License- Required
- typing and Computer Skills - Required
- BLS-Required
- Previous ambulatory care experience - Highly desired
