Position Summary:

As the RN Care Coordinator, you would be responsible for  providing case management and outreach services to assigned cases. 

Hours for this Position: Monday-Friday 8am-5pm

More Insight of Daily Responsibilities: 

• Complete assessments and develop/implement appropriate care plans based off of findings
• Provides information regarding benefits and community resources available 
• Identifies potential risks and barriers
• Communicates and coordinates with family members, physicians, and other healthcare professionals
• Completes face-to-face visits in order to monitor changes and track progress

What We Look For:

• Clear and active RN license in state of IL
  
• 3+ years of Case or Disease Management experience
• Able to travel locally (must have valid driver's license and auto insurance)
• Pediatric experience preferred
