Job Description
Are you an experienced RN Case Manager looking for a new opportunity with a prestigious healthcare company? Do you want the chance to advance your career by joining a rapidly growing company? If you answered “yes" to any of these questions – this is the position for you!
Daily Responsibilities:
Provide long-term care management to a frail, economically disadvantaged, culturally diverse older adults, disabled adults and caregivers in order to help them remain safely at home and to prevent premature institutionalization.
Assess new clients and determine appropriateness for the program, certify the client as nursing home eligible, conduct assessments and reassessments for identification of client’s medical, physical and psychological needs.
Ensure client’s needs are met through written care plan approved by the program supervisor and discussed verbally with the client and/or his representative.
Provide ongoing health education to non-clinical staff members.
Present cases at and participates in weekly team case conference per guidelines.
Discuss during case conference complex health care issues for teaching purposes.
Develop and implement care plans for assigned caseload.
Requirements:
Must be Registered Nurse with clear and active CA license
Must have 2+ years of clinical experience
Must have 3 + years of experience with health assessments, level of care criteria, care planning, and case documentation.
Previous work with older adults and conducting home visits highly desirable.
Previous work with mental health and/or substance abuse issues desirable.
Ability to work as a part of a team, while at the same time performing job independently
Must have strong computer and typing skills
Hours for this Position:
Monday – Friday 8-5
