Job Description
Daily Responsibilities:
- Telephonic office position
- Assessing patients’ ADLs (Activities of Daily Living) and doing HRAs (health risk assessments) and care plan reviews to assess the need for services that allow members to remain out of the hospital
- Working with the ABD (aged, blind, disabled) population
Hours for this Position:
Monday through Friday 8am-6pm and some Saturdays

Qualifications
- LVN license
- 2+ years of LTC or Home Health experience
- 2+ years experience working with the Aged, Blind and Disabled population (SSI patients on disability
- Experience coordinating services and knowledge of community/government-funded services in the area
- Bilingual English/Spanish highly preferred
