Job Description
Are you an experienced LVN Service Coordinator looking for a new opportunity with a prestigious healthcare company? Do you want the chance to advance your career by joining a rapidly growing company? If you answered “yes" to any of these questions – this is the position for you!
Daily Responsibilities:
• Will be completing “2060 Assessments,” assessing patients’ ADLs (Activities of Daily Living) and need for services
• Talking on the phone with the ABD (aged, blind, disabled) population
Hours for this Position:
• Mon-Fri 8:00-5:00, possibility of OT and/or Saturdays

Qualifications
• LVN license
• LTC or Home Health experience (approx. 2 years)
• Look for experience working with the Aged, Blind and Disabled population (SSI patients on disability)
• Experience assessing ADLs (have used the 2060 form)
