Job Description

• Assist members with care coordination and case management

• Help members assess, plan and establish goals and coordinate a plan of care, acting as an advocate.

• Determine care coordination and case management needs for all referred members

• Make appropriate referrals

• Document progress towards meeting goals and resolving problems

• Coordinates care and services with the Account manager, Complex RN Case Manager, and member, family members as appropriate, PCP, Specialist, and Facility/Vendor Providers


Qualifications

• Clear and active RN License OR unrestricted MSW license

• Bachelor’s Degree (If RN)

• 3-5 years of care management experience

Hours for this Position:

Monday-Friday daytime hours

40 hrs a week
