Job Description
Position Summary:
Provides a deliberate patient and family centered partnership with the healthcare team that facilitates a seamless patient experience between the Inpatient and Outpatient settings. The Care Coordinator works to maximize the patient’s health outcomes and the patient experience by identifying patient and family needs, completing pre-visit planning, supporting the clinic visit and providing closure to the clinic visits.
Major Job Duties:
- Coordinates care across the health care continuum within designated sub-specialty population(s). Serves as single point of contact by facilitating and overseeing transitions in care within and between care settings, providers, and services.
- Manages patient care in-between clinic visits including communication with primary and referring providers.
- Promotes safe, satisfying patient centered care as defined by the patient and family centered care model.
- Facilitates proactive disease management/preventative health care using evidenced based practice.
- Completes visit closure and summarizes visit.
- Completes pre-visit planning to optimize patient visit. Ensures that appropriate authorization for
- procedures/devices and medications have been obtained prior to services needed
- Demonstrates ability to provide care or service adjusting approaches to reflect developmental level of population served. (as applicable)
- Performs other duties including Performance Goals developed by manager and employee and reported in the employee’s Performance Review as part of UMPhysicians’ Performance Management tools
Qualifications
What We Look For:
- Required experience with Chemotherapy and cancer infusions.
- Experienced with treatment plans.
- Prefer Gynecologic Cancer experience but not mandatory
- MN RN License
- 3 years of clinical experience – preferably some clinic setting experience
- Nursing assessment and evaluation skills and abilities.
- Knowledge of standards and clinical practice for area of care coordination
- EMR experience
- Ability to work within a team and advocate for patient and family needs
- Computer skills
- Patient education and pro-active disease management experience.
- Excellent assessment and care coordination skills.
- Strong customer focus to accommodate patients.
