Job Description

Are you an experienced Registered Nurse with working knowledge of charting, patient education, and care coordination looking for a new opportunity with a prestigious hospital? Daily Responsibilities: 

· You provide deliberate patient and family centered partnership with the healthcare team that facilitates a seamless patient experience between the Inpatient and Outpatient settings.

· The Registered Nurse Care Coordinator works to maximize the patient’s health outcomes and the patient experience by identifying patient and family needs, completing pre-visit planning, supporting the clinic visit and providing closure to the clinic visits

· You work to develop and support a coordinated plan of care between visits to promote a smooth transition for patients and family across the healthcare continuum that instills a sense of trust and patient advocacy.


Qualifications

• Current MN RN License– Required

• BLS– Required

• Patient education and proactive disease management experience– Required

• Excellent assessment and care coordination skills – Required

• Typing and Computer Skills – Required

• BSN or MSN Specialty certification or training knowledge of case management – Highly Desired

• Patient advocacy experience – Highly Desired

• Skilled in using EPIC – Highly Desired


Additional Information

Hours for this Position: Monday-Friday, 8:00am-5:00pm
