Company Job Description/ Day to Day Duties:

Summary: 

  • The Licensed Practical Nurse is a licensed nurse responsible for collection of data to be used in patient assessment, contribution of patient information to be used in planning patient care, implementation of nursing interventions listed in the plan of care, and evalution of patient responses to nursing interventions. 
  • The LPN also participates in patient teaching, discharge planning, rehab, and assists with coordination of other activities of ancillary nursing personnel while maintaining standards of professional nursing practice and the established policies and procedures of the hospital.
  • Collects data, plans and implements the nursing care of residents within the skilled nursing setting with the supervision of a Registered Nurse. 
  • Responsible for ensuring continuity of care of the residents between shifts by providing direct care as well as supervising the care given by CNA's and support staff members.  Participates in resident and family teaching. 
  • Maintains role as resident advocate with a focus on the facility's mission.

 

Information about Hospital: 122,600 square foot facility. There are 165 beds including 77 beds dedicated to inpatient rehabilitation and 88 beds for long-term care. The facility contains new rehabilitation gyms with state-of-the-art rehabilitation equipment. In addition, an Activities of Daily Living apartment gives residents the chance to practice their routines before they return home. Modern living and dining areas are designed to offer space for residents to engage with one another.

Qualifications

  • At least or close to 1 year of experience as an LPN
  • Must have active MI LPN license
  • Solid Navy Blue Scrubs required (no reimbursement)
