Job Description

The Director of Clinical Development will work collaboratively as a part of a leadership team to develop and implement key elements of a standardized Model of Care (MoC) in participating skilled nursing facilities. This MoC will be supported by the development of a remote clinical coordination system designed to facilitate timely clinical intervention and support for patients at risk of hospitalization. The primary role of the Director of Clinical Development will be to identify and implement innovative strategies and clinical solutions to support project efforts. This will include developing standardized care paths, transition processes, communication protocols, support systems function and management of the clinical coordination team.
An advanced clinical skill set specific to the nursing home environment is essential. This is a 1.0 FTE position with competitive salary and benefits. This position requires a combination of work from an office in Roseville, MN and regular travel to participating locations within the Minneapolis/St. Paul, MN metro area. This position will work collaboratively with a contracted project management team and report to the CareChoice Medical Director

  • Working knowledge of post/acute care EMRs, particularly Epic, Matrix, Point Click Care

Working knowledge of State and Federal regulations in the nursing home environment.

  • Practical working knowledge of computers and related office software (MS Office, SharePoint) and equipment

Qualifications

Education: Minimum Bachelor of Science in Nursing (BSN) from an accredited university; Master of Science in Nursing (MSN) or higher level preferred; Registered Nurse (RN) with current unencumbered licensure with the Minnesota Board of Nursing.
Experience: Five or more years of director-level experience managing clinical quality in a skilled nursing home setting.
