Job Description

The Bundled Care Coordinator plays a pivotal role in ensuring seamless transitions from hospital to home for post-surgical bundled care (ex: MSK) patients. This position is designed to be integral in supporting transitions of care, with a focus on enhancing the patient experience and facilitating efficient discharge from hospital to community programs. This role assists with system navigation from hospital to community and ongoing community management. This position supports coordination of client services by gathering information, liaising with clients and families, and facilitating communication among care team members. This role does not involve independent care planning, but rather seeks input, direction, and care plans from regulated health professionals and ensures these plans are implemented effectively. Working collaboratively with an inter-professional team, both internal and external to Halton Healthcare, including but not limited to Home Care Agencies, Peer Hospitals, Community Physiotherapy Clinics, and Regional Partners. 

 The Bundled Care Coordinator will support the Surgical Transitions Remote Care Monitoring program (Total Hip, Knee, and Shoulder pathways) by monitoring the patient dashboard. This includes following up on alert flags and directing the concern to a regulated health professional to assess and adjust the care plan.   

Qualifications

  • An undergraduate degree or diploma/certificate in a relevant health discipline field is mandatory 

  • Membership with the appropriate regulatory body in Ontario-preferred  

  • 2-5 years of recent work experience in a hospital setting with specific experience in surgical services and rehabilitation 

  • Experience working within an interdisciplinary team, particularly in acute and community settings.  

  • Demonstrated experience in rehabilitation, particularly in the areas of post-surgical rehabilitation, education, and the care of the elderly.  

  • Evidence of program and resource development skills, including knowledge of evaluation methodologies.  

  • Experience in care coordination, case management, or resource management is highly valued.  

  • Demonstrated to work independently and collaboratively with interdisciplinary teams, family members, support networks, and a variety of formal and informal service providers.  

  • Strong leadership skills, including problem-solving, critical thinking, conflict resolution, and negotiation abilities. 

  • Excellent interpersonal, communication, organizational, and decision-making skills. 

  • Demonstrated presentation skills for staff and patient education.  

  • Exceptional time management and stress management skills. 

  • Demonstrated conflict resolution skills.  

  • Comfortable working in an evolving program with changing responsibilities over time. 

 

  • Proficiency in electronic medical record (EMR) systems; experience with Meditech and CHRIS (preferred).  

  • Experience with remote care monitoring programs (SeamlessMD preferred).  

  • Computer literacy and basic proficiency are required.  

  • Experience with excel for data collection and monitoring.  

  • Experience with Power BI and PICIS (preferred).

Hours: Monday - Sunday; Various; All Shifts; 8:00-16:00 (Hours are subject to change based on operational needs)

Salary:  $40.011 to $56.425 per hour
