Job Description

Principal Duties and Responsibilities

  • Oversees all program functions for GPD services on Clinical Team / SITH Team and/or Bridge Team
  • Supervises a team of GPD Case Managers and provides staffing to GPD Case Managers
  • Reviews and approves funds requests for GPD clients
  • Monitors relevant program budgets
  • Works with team members and other agencies to help identify, evaluate, and assist potential and current clients
  • Conducts assessments and client intakes and maintains a caseload as warranted
  • Accurately completes and submits monthly and quarterly reports on time
  • Monitors client case management files and updates progress notes within 48 hours in those files
  • Updates and maintains client information in the HMIS database
  • Attends all relevant community and CoC meetings
  • Locates appropriate resources for clients and makes appropriate referrals

Qualifications

Education/Experience/Skills Required

  • Bachelor’s degree in a related field; BSW or MSW preferred
  • Licensed in Social Work and/or credentials are strongly preferred (required for Clinical team)
  • Willingness to be flexible and cooperative
  • Ability to work independently with strong problem-solving skills and also effectively collaborate with the GPD team and other teams and departments with the agency
  • Demonstrated strong understanding and comprehension of the GPD program and community resources
  • Demonstrated strong interpersonal skills and ability to effectively communicate
  • Demonstrated strong time management skills and ability to prioritize
  • Knowledgeable of and able to provide referrals to other community resources
  • Knowledgeable of the factors associated with homelessness, substance abuse, and mental illness
