POSITION SUMMARY: Work Site Location is WRNMMC. The Social Worker will support a patient workload via Telehealth or remote video counseling within the Behavioral Health domain. The incumbent will see patients primarily by Telehealth modality and in person when necessary.

DUTIES: The Social Worker will provide the following services and in strict compliance with standing orders and protocols set forth by the WRNMMC, and IAW JC guidelines to include, but will not necessarily be limited to the following:

* Screen and interview patients, perform evaluations using expert knowledge of general and specific concepts, principle, procedures, and practices related to psychological counseling.

* Provide individual, family and group psychotherapy, and couples therapy.

* Provide screenings, clinical assessments, treatment, case management, consultation, referral and education.

* Assess and resolve complex social, economic and psychosocial problems that may impact on the service members’ and their families’ medical treatment as outpatients, inpatients or while the service member is being evaluated for medical discharge from the service by providing behavioral health direct services, to include screening, assessment, treatment, referral, consultation and education and in conjunction with DoD BH-CPGs.

* Conduct psychosocial evaluations and provides therapeutic interventions including crisis counseling to individuals, groups and families as needed to facilitate behavioral health clinical care management and optimize care.

* Evaluates the individual and family systems as well as assesses patient’s functioning within work, family and routines of daily living and identifies areas needing continued support, resources, and treatment in order to assist patients

* Serves as a behavioral health referral resource and subject matter expert in behavioral health- related programs to the MTF staff, medical staff in primary care clinics and other on-post medical settings.

* Provides services for high-risk populations (service members pending medical discharge due to physical and or behavioral health injuries/illnesses, service members who were wounded as a result of their military duties, and the families of service members killed in action, etc.)

* Develops, implements, and maintains partnerships with Soldiers, family members, other health care providers and community resources, as applicable. Develops comprehensive treatment care plans in accordance with behavioral health care standards.

* Coordinates services for those service members or patients who move out of the local area to ensure continuity of care. Facilitates transfer of the case to the gaining facilities’ behavioral health care case management resources, as appropriate

* Serves as an advocate for service members and patients as needed, assists with navigating health care systems in obtaining appropriate referrals. Some of these contacts/referrals may include communicating with physicians, nurses, behavioral health professionals, prevention specialists, family members, etc.

* Responsible for the completion of individual workload accountability in accordance with clinic, medical treatment facility and program requirements

* Abide by all applicable MTF by-laws, JC, DoD, with regard to Utilization Review and Quality Improvement directives including, but not limited to, in-service training, peer review, performance improvement, records maintenance, performance evaluations, and release of information. Participates, as appropriate, in departmental Quality Assurance (QA) programs and adheres to all safety regulations at all times. Provides technical advice and professional opinions upon request from the MTF Commander, and/or C, Department of Psychiatry, Service OIC, or other assigned supervisors.

* Attend and participate in patient care reports, patient care conferences, team conferences, professional staff conferences and any other appropriate professional activities only to the extent that such attendance and participation is relative to assigned cases and/or performance of contract services as determined by the MTF Commander, Department of Psychiatry, Clinic OIC, and other assigned supervisors, as required and as limited by this task order (i.e. within the 50 mile radius of the NCR).

* Present patient cases in regularly scheduled case staffing meetings to discuss status and updates to cases under the direction of the C, Department of Psychiatry, Clinic OIC, and other assigned supervisors, as required.

* Maintain complete and accurate records and documents all patient contacts/services provided in accordance with professional standards. Enters patient clinical and administrative data as well as utilization and other data into automated systems and/or paper records and create reports from these data in accordance with local policies and procedures. The HCP shall complete medical charting within 72 hours of patient visit IAW with JC guidelines.

* Demonstrate excellent written communication skills to perform accurate documentation, both written and electronic, of all activity in accordance with requirements.

* Can demonstrate good time management skills. Preferred experience with Telehealth visits

* Other: Ensure compliance with standard of care and practice in accordance with all established policies, procedures, and guidelines used in the medical treatment MTF and IAW JC guidelines. Provides care within ethical and legal boundaries. Completes orientation and competency verification programs in accordance with unit guidelines. When assigned, will serve and participate in committees, functions and other meetings. Provide relevant and timely information, and assists with decision-making and process improvement. Will report to the Director of Social Services. Also may serve as preceptor for Clinical Social Worker preceptorship programs. As required, participate in customer service initiatives and medical readiness activities, designed to enhance health services and Telehealth practices.

* Education: The Social Worker will have graduated from an ASWB accredited Master of Social Work (MSW) program.

* Licensure: Current unrestricted license Social Worker to work independently without clinical supervision, in at least one U.S. State, District of Columbia, Commonwealth, territory or jurisdiction. All licenses shall be unencumbered and remain in effect during employment.

* Experience: Have a minimum of two (2) years within the past five (5) years in the independent practice of clinical social work in child, adolescent and family therapy. Preferably worked with DoD beneficiaries.

* Certifications: Possess and maintain a current BLS certification (American Heart Association – Healthcare Provider).

* Additional Required Knowledge: 1) Shall be able to read, write, speak and understand English well enough to effectively communicate with all patients and other health care providers. 2) Shall be knowledgeable and possess skills necessary to provide care appropriate to any age-related needs of the patients. The Contract SW shall demonstrate knowledge of the principles of growth and development appropriate to the appropriate patient population served: Children, Adolescents, Adults, and Geriatric (>65). 3) Assess, coordinate, facilitate, deliver and evaluate care for Children and Adolescents primarily, and adult patients if needed using the appropriate process. 4) Knowledge of a wide range of medical disorders/conditions, diagnostic studies and treatments related to prevention, and detection of illnesses in adults, is desirable. Knowledgeable in professional care theories, principles, practices, and procedures to perform nursing assignments of moderate difficulty and experience in assessing adult and geriatric patients, as appropriate. 5) Knowledge of computer operations and proficiency in the use of basic word processing, data entry and automated medical records (Preferred experience with DoD’s AHLTA/CHCS/Essentris). 6) Ability to work well with others in a team environment. 7) High degree of interpersonal skills when dealing with all levels of customers and personnel within the correspondence section and at the Command level.

SCHEDULING & STAFFING:

* Duty Hours for this position are 0730 to 1600 Monday through Friday, to include uncompensated lunch break (40 hours per week).
