Job Description

The Social Services Designee/Activities Director is responsible for providing leadership for working with residents in the nursing home by identifying their psychosocial, mental and emotional needs along with providing, developing, and/or aiding in the access of services to meet those needs including; coordinating pre-admission screening, admissions paperwork as well as organizing and conducting services that promote personal growth and family involvement in the overall care of the resident. This position aids residents and their families with personal and environmental difficulties, which may predispose illness or interfere with obtaining maximum benefits from skilled care.  Furthermore, this position supports the extended stay and discharge process by providing information and referral services to residents and families. In addition, to being responsible for directing the development, implementation, supervision and ongoing evaluation of the activities program.



  • Desire Bachelor’s Degree in Social Work or a Bachelor’s Degree in a human services
    field from an accredited academic institution, including, but not limited to:
    sociology, special education, rehabilitation counseling, psychology.  Substitution of experience in lieu of education per community policy.

  • Knowledge of long-term health care Federal and State law, rules and regulations.


  • Three (3) to five (5) years of supervised social work experience in a health care
    setting, working directly with individuals desired. 

Certificates, Licenses, Registrations

  • Maintains current licensure with MN Board of Social Workers as required by
    current state licensure requirements
  • Participates in staff training programs.
  • Participates in Social Service policy development, orientation and in-service training of
