Job Description

In this exciting full-time Grade 1 Social Worker role, based in Richmond, you will be responsible for providing social work services to the acute general medical ward. You will be part of the acute social work team and report to the social work team leaders and allied health manager.

 You will make an impact by:

  • Striving for patient and family inclusive practice in all stages of care.
  • Engaging positively with all other colleagues to achieve best clinical outcomes.
  • Developing the social work service both internally and externally via working in a collaborative model of practice.
  • Providing an exceptional patient–centred care experience.
  • Being a patient advocate, whilst understanding the needs of the organisation.

To be successful in this role…

You will be a tertiary qualified social worker, with experience in psychosocial assessments, adjustment to illness, counselling, discharge planning and community liaison in the acute hospital setting.

Experience in end-of-life care, residential aged care placement, carer stress, capacity and decision-making issues, as well as VCAT/Guardianship and NDIS applications and processes, will be highly regarded.

You will have:

  • Strong interpersonal skills, working closely alongside the interdisciplinary team on the ward.
  • Previous experience in an acute hospital setting (highly desirable).
  • Well-developed organisational and planning skills.
  • Be able to interact and collaborate with all members of a multidisciplinary team. 
  • Have sound theoretical knowledge of current social work practices, particularly in the areas of health and disability.
