Job Description
Epworth Rehabilitation is seeking registered nurses to work Part time -0.6EFT with acute or rehabilitation experience to work across all Epworth Sites. Working within a multidisciplinary team environment, you will join a patient-centred team caring for patients who are recovering from surgery or illness or needing support to maximise their independence.
Rehabilitation Assessor role will be to facilitate coordination and continuity in the provision of patient assessment, referral and transfer from acute hospitals to Epworth HealthCare rehabilitation services. It is expected that a Rehabilitation Assessor will act as an ambassador of Epworth HealthCare and Epworth Rehabilitation through:
- The process of reviewing referrals from the acute to rehabilitation care settings in Epworth HealthCare
- Making clinical decisions in relation to identifying the best rehabilitation options for each patient, including identifying individual goals to achieve in rehabilitation
- Developing collaborative relationships with the key stakeholders (physicians, allied health, nursing) from the acute, community and the rehabilitation settings
- The position is responsible for ensuring timely assessment for patients moving from acute sites to Epworth Healthcare Rehabilitation Units.
Rehabilitation helps patients recover from an illness, injury or surgical procedure, manage a chronic illness or prevent further injury. This is a rewarding opportunity to work in a team motivated to assist patients in returning to their daily lives.

Qualifications and Experience Required
Inherent Requirements for the Role:
- Current nursing registration with AHPRA (Australian Health Practitioner Regulation Agency).
- Valid driver's license.
- Strong communication skills, both verbal and written.
- Ability to work autonomously or within a small team.
- Capability to work efficiently under pressure.
- Physical ability to walk long distances, as daily movement between wards and facilities is required.
- Excellent interpersonal skills, with the ability to build rapport with patients and healthcare professionals.
- Strong organizational and time management skills.
- Proficiency in using electronic health record systems.
- Ability to prioritize and manage a diverse caseload effectively.
Experience Required:
- Minimum of 3 years' experience in acute or rehabilitation nursing.
- Demonstrated knowledge of current rehabilitation practices and evidence-based care.
