Job Description

Epworth Rehabilitation is seeking to appoint a dynamic and experienced Grade 3 Social Worker to provide comprehensive social work services to patients in both an inpatient and outpatient setting across the rehabilitation sites at Richmond, Hawthorn and Camberwell. This role would primarily be based at Epworth Camberwell.

This is a part time, fixed term position of 30.4 hours per week, until May 2025. 

Responsibilities will include undertaking comprehensive psychosocial assessments, providing supportive counselling, liaising with key stakeholders and discharge planning. Commensurate with the Grade 3 role, responsibilities will also include staff and student supervision, clinical leadership and participation in senior forums within the Rehabilitation and Mental Health Division. The Social Work Department has a high commitment to supervision and professional development, as well as to engagement in quality activities and research.

You will work in an energetic and cohesive clinical team that strives for excellence in patient care. 

You will make an impact by:

  • Undertaking comprehensive psychosocial assessments and complex discharge planning utilising your specialist knowledge in the areas of aged care, VCAT, NDIS, family violence and elder abuse
  • Striving for patient and family inclusive practice in all stages of care
  • Providing leadership within the Rehabilitation Social Work team and wider Rehabilitation team at Camberwell
  • Providing high quality supervision to staff and students
  • Leading quality improvement and/or research projects
  • Demonstrating flexibility and adaptability in a dynamic clinical environment

To be successful in this role you will have:

  • A minimum of seven years relevant social work experience in a hospital, rehabilitation or other healthcare setting
  • Knowledge in the area of the aged care sector would be highly desirable
  • Ability to manage a complex caseload of both inpatients and outpatients and facilitate group work 
  • Extensive skills, enthusiasm and commitment to your work and embrace the opportunity to contribute to the positive culture at Epworth HealthCare
  • Well-developed expertise in assessment, intervention and discharge planning

  • Bachelor or Master of Social Work
  • Eligibility for membership of the Australian Association of Social Workers (AASW)
