Job Description

What You Get To Do:

As the Health Services Professional (HSP) your primary role is anything related to medical supports of participants in our programs.  This includes, but is not limited to: 

  • Ensure routine and non-routine medical appointments are scheduled to uphold the health and safety of participants. 
  • Assist individuals to medical appointments. 
  • Ensure medical paperwork is processed and documentation is put in electronic records. 
  • Ensure any needed follow up to medical appointments is scheduled and completed.  Check in medications.
  • Deliver medications to homes on a routine basis. 
  • Be primary point of contact with participant’s medical providers and facilitate sharing of information from medical providers to staff and teams. 
  • Ensure medical supports are compliant with Kentucky regulations. 

Hours:

  • Hours are generally Monday through Friday (8:30am - 4:30pm); however;  must be flexible to accommodate participants, staff, and home needs.

    This position assists with taking calls after hours.
