The Support Services Coordinator (SSC) is a pivotal position in the home. This individual performs all of the responsibilities and duties of the Direct Support Professional (DSP), and in addition holds primary responsibility for areas of health care/nutrition and household management for the individuals served. In these areas, the SSC is involved directly in all assessments, planning, and implementation of services and supports. The SCC will assist in direct training, follow up, coaching with service staff and keeping the Program Director informed as necessary.
The person in this position has the responsibility further to coordinate a variety of services including but not limited to: finances, recreation, health services, family involvement, educational/vocational programs, and all other supports/services necessary to ensure a good quality of life. The SSC is responsible to assist the Program Director in maintaining consistency in the provision of supports and services to the individuals served, and in communicating with staff and team members. This person is also responsible for carrying out the responsibilities and duties of the Direct Support Professional during all direct service shifts, according to Individualized Service Plans (ISP), as well as any addenda to this job description. The SSC may spend up to twenty percent (20%), of their scheduled time on additional household duties on behalf of individuals supports.
- Minimum 18 years of age
- High school diploma/GED required
- Two (2) Years of experience in the field of Intellectual/ and/or Developmental Disabilities
- Must have a valid driver’s license and an acceptable driving record
