Responsibilities:

• Ensure that each individual receives quality, individualized health care management
• Conducting routine site visits, checking vitals, doing assessments, counseling of medication administration, and documenting progress
• Act as a health/medical resource for staff and assist in training staff in the medical arena
• Conduct audits and reviews of medications
• Ensure that the company complies with state requirements and organizational policies regarding medical care and records

Requirements:

• Must be a licensed practical nurse, currently licensed in the State of Minnesota

• Preferably with experience in service for persons with developmental disabilities or long-term care

• Basic computer skills

• Strong observational, organization & communication skills

• Ability to work independently & as a team member

• Experience with physical, psychological &/or developmental disabilities a plus

• Valid driver license with acceptable driving record.
