Job Description

  • Case Management for children in foster care and adults with developmental disabilities
  • Monitoring Host Home Providers and assuring all services and supports are coordinated
  • Facilitate Interdisciplinary Teams
  • Physical Site Monitoring 
  • Opportunity to train Providers and Staff on ISP's
  • Provide ongoing coaching and development of employees
  • Complete and update Safe home studies
  • Complete placement logs, waivers and development plans
  • Send monthly reporst to DHS.

Qualifications

  • Bachelor's Degree in the social or behavioral sciences. If an individual has a Bachelor's degree in a non-related field, must have at least two (2) years experience supervised by an  individual with a Master of Social Work or other Master's degree in the social or behavioral sciences
  • Experience in developing and implementing individual programs
  • Must have supervisory experience
  • Must have good decision-making, time management and communication skills
  • Must be responsible, mature and flexible
  • Valid driver's license with acceptable driving record
  • Current auto-liability insurance
  • Experience serving persons with Developmental Disabilities is a PLUS
