Job Description

The Community Support Coordinator is responsible for working cooperatively with the Program Manager and other staff in developing, implementing, and evaluating the outcomes of community living supports provided to individuals with developmental disabilities.  The Community Support Coordinator is expected to contact the Program Manager regarding any program or personnel issues within the home as well as for emergency household, medical, or maintenance needs in the home.

The Community Support Coordinator is responsible for providing and coordinating direct services and healthful lifestyle supports to individuals with disabilities.  These living supports include, but are not limited to:  supervising, teaching, and assisting the individuals in every aspect of their lives according to their Individualized Support Plans (ISP) with the goals of fostering increased independence, choice, and empowerment. They will assist with maintaining a clean, safe living environment, and assist in meal preparation and completion of various household duties. In addition, the Community Support Coordinator ensures the implementation of routine household activities, individuals' services and supports, and communication.

This is an entry level/lead direct support position. 

Starting wage range of $10.91- $11.30 per hour.

Qualifications

  • Must be at least 18 years old
  • Minimum of one (1) year experience required in the field of Developmental Disabilities or a Bachelor's degree in Human Services or related field
  • Valid driver's license with acceptable driving record
  • Current auto liability insurance
  • Reliable transportation
  • Acceptable criminal record
