Job Description

The nurse will assure that each individual receives quality, individualized health care management resulting in optimal health.  Health care will be provided to individuals in a manner that is normalizing, sensitive to their needs, and responsive to their health care choices with respect to their dignity.

The nurse will provide care in accordance with the Ohio Nursing Licensure Laws and Regulations.

To assess each individual’s health care needs initially upon admission and throughout placement to ensure they are identified and met.

To ensure that medical care and documentation meet state regulatory standards and organizational policy.

To cooperate with the Directors and Program Directors in coordinating efforts, services and knowledge to maximize the benefit to individuals.

To ensure that all program staff receive on-going information, mentoring and guidance in providing for the health care needs of the individual.

To represent the organization in a positive, professional, responsible manner to staff, guardians, other health care professionals, case managers and team members.

Be an involved member of the interdisciplinary team and actively participate as deemed appropriate by the supervisor.

Communicate with involved team members regarding medical aspects of a particular individual as requested by the team or as needed, ensuring continuity of care.

Obtain vital signs and carry out other necessary procedures for specific individuals per physicians' orders (e.g., G-tube feedings).

Qualifications

Must be a licensed LPN in the state of Ohio.   Minimum of 2 years education and experience working with individuals with disabilities or other long-term care is preferred. The individual must be professional, flexible, self-motivated, dependable, and able to work cooperatively with staff, Individual team members and outside agencies .All employees in this position must have a valid driver’s license and an acceptable driving record, as defined by our insurance carrier.  An employee hired for a position that requires the employee to drive his/her own car is responsible for his/her own automobile insurance.
