At Dimensions Home Health Care, our mission is to inspire hope and improve the quality of life for those patients entrusted to our care.  We provide home health care under Medicare and Private Insurance to  patients in their home;  be it a single-family home, apartment building or assisted living.

Our Registered Nurses,  Therapists,  and Social Worker coordinate care in Lake & McHenry County in Illinois and Kenosha, Racine & Walworth County in Wisconsin.We work with hospitals, skilled nursing providers and primary physicians to continuously assess medical conditions; ensure medication compliance by educating patients and caregivers on the purpose, frequency, proper administration, and possible side effects of medications; all while maintaining a level of care, compassion and consistency that dramatically improves the quality of life of our patients.

Provide hands-on personal care and assist with self-care skills to clients in their homes.

• Develop a successful interpersonal relationship with our clients.
• Assist our clients with activities of daily living, including bathing, light housekeeping, feeding and preparing meals.
• Follow infection control and safety measures when caring for clients.
• Recognize and report changes in client's condition and safety to care coordinators
• Positions Available:

• Days 7am to 7pm Saturday Sunday
• Nights 11 pm to 7 am. Saturday and Sunday

• Days 7 am to 7 pm Monday through Thursday

• At least one year of professional experience as a Caregiver and completion of an CNA course
• Licensed CNA in the state of Illinois
• Two forms of ID such as Driver's License, State ID, Passport, Social Security Card
• General Physical Examination and TB Test within last year or Chest X-Ray
• CPR Certification
• Two Professional References
