DC Primary Care Association is a 20-year old nonprofit health equity and advocacy organization dedicated to improving the health of DC’s vulnerable residents by ensuring access to high quality primary health care.

Our membership currently includes 15 community health centers and community-based organizations located in the District of Columbia and the Maryland suburbs. Between them, our members own and operate nearly 60 health care delivery sites that serve approximately 200,000 residents. 

Job Description

DC Primary Care Association’s (DCPCA) member health centers provide comprehensive primary care to 1 in 4 Washington D.C. residents, reaching a total of over 1 million visits a year. DCPCA’s Quality Improvement (QI) team provides member health center staff with the training and tools to strengthen and improve access to care, operational excellence, and health outcomes. Our member health centers provide high-quality, comprehensive, affordable, culturally-competent medical, dental, and behavioral health care.

The Primary Care Consultant is part of an innovative department at DCPCA, supporting the improvement of clinical and patient outcomes through implementation of evidence-based services. This is a dynamic position with a high degree of independence and flexibility, allowing space for an entrepreneurial spirit to pursue their passions. DCPCA’s QI department supports health centers through technical assistance, face-to-face trainings, webinars, dissemination of resources and peer-to-peer connections. Our work is data-driven, evidence-based, and grounded in QI principles. We seek solutions to systemic healthcare obstacles for vulnerable populations.

Essential Duties and Responsibilities 

Provide Technical Support and Training to Health Centers:

  • Plan, coordinate, facilitate, and evaluate learning sessions/trainings for health center staff in collaboration with project partners including curriculum design, agenda setting, and trainer identification.
  • Serve as a resource for members on key content areas – connecting health center staff with the tools and resources they need to reach their care goals.
  • Provide technical assistance to member health centers around identified needs.
  • Work intensively with health center teams participating in assigned projects to refine delivery system and implement best practice concepts.
  • Design, develop, and implement member assessments around clinical performance and TA needs to identify opportunities for improvements and possible solutions.
  • Lead in the coordination of health center Peer Groups, including planning meeting logistics, communicating with members, preparing content for meetings, and directing follow-up coordination.

Knowledge Contribution:

  • Work with local and national agencies, independent subject matter experts, and member clinics to identify best practices and evidence-based practices.
  • Conduct research to expand the QI department’s subject matter expertise.
  • Disseminate resources to health center staff and regional bodies in pursuit of project-specific goals.
  • Prepare project reports, memos and maintain project records and statistical information.
  • Support administration of program grants and deliverables including coordination of project activities, tracking deadlines, reporting activities, and convening the stakeholders needed to ensure that projects and products are of the highest quality.
  • Attend relevant stakeholder and advisory meetings to support overall program implementation and evaluation.
  • Other duties as assigned.

 

Qualifications

Must haves:

  • Knowledge of subject matter areas of focus:
    • Patient Centered Medical Home (PCMH), team-based care and care coordination
    • Quality measure collection, analysis, and reporting
    • Chronic disease prevention and management, in particular diabetes and cardiovascular disease
  • Master’s degree or Bachelor’s degree in relevant field with 5 years of relevant work experience equivalent.

·       Ability to balance multiple priorities and creatively use available resources to drive transformation.

·       Excellent training and meeting-facilitation skills

·       Ability to build relationships with diverse stakeholders.

·       A customer-service orientation, with enthusiasm for finding solutions to complicated questions.

  • Experience working with or in primary care settings/clinics.
  • Excellent written and oral communication skills.
  • Proven experience with project management, in particular demonstration of ability to organize various activities and goals, manage multiple projects simultaneously, plan strategically, and see projects through to completion.
  • Strong relationship-building skills, with proven ability to develop effective relationships and work collaboratively with healthcare providers and diverse stakeholders (government, community, and business environments).
  • Team-player, ability to coordinate work collaboratively and resolve conflicts.
  • Effective analytical and problem-solving skills, with the ability to quickly gather information (quantitative and qualitative), identify trends, and propose solutions.
  • High degree of proficiency with MS Office products, including Word, Excel, PowerPoint, and Outlook.

  • Experience in a primary care setting
  • Experience in additional focus areas for the QI team is helpful, but not required:
    • Cancer screening (colorectal, breast, and cervical)
    • Behavioral health integration
    • Substance use disorder treatment, including opioid abuse
    • Social Determinants of Health (SDOH)
    • Trauma-Informed Care
    • Value-based payment and population health management
    • Best practices for use of available EHR, HIT, and HIE tools, including Meaningful Use (MU)

ADA Specifications

This position will be based in an office in Washington D.C. but will require travel to health centers throughout D.C. This position is primarily sedentary and may require some bending and lifting.

DCPCA is a fast-paced, high-energy organization with a very ambitious agenda and a staff that is highly motivated. This job may experience high work demands under tight timelines.
