DC Primary Care Association is a 20-year old nonprofit health equity and advocacy organization dedicated to improving the health of DC’s vulnerable residents by ensuring access to high quality primary health care.

Our membership currently includes 15 community health centers and community-based organizations located in the District of Columbia and the Maryland suburbs. Between them, our members own and operate nearly 60 health care delivery sites that serve approximately 200,000 residents. 

Job Description

The Health Center Operations Consultant is part an innovative and growing department at DCPCA, supporting the improvement of clinical and patient outcomes through implementation of evidence-based services. The Health Center Operations Specialist provides trainings, technical assistance, and collaborates with other organizations to optimize clinical practice and improve quality performance.

DCPCA’s QI department supports health centers in providing high-quality care through technical assistance, face-to-face trainings, webinars, dissemination of resources and peer-to-peer connections. Our work is data-driven, evidence-based, and grounded in QI principles. We seek efficient and proven solutions to systemic healthcare obstacles for vulnerable populations. We leverage this knowledge to connect resources and best practices to health center staff and patients.

Our QI department provides training and technical assistance to health center administrative staff on a range of care topics. For this role, we are seeking someone with experience with primary care operations, specifically in the following areas:

  • Patient Centered Medical Home (PCMH), team-based care and care coordination
  • Chronic disease prevention and management, in particular diabetes and cardiovascular disease
  • Quality measure collection, analysis, and reporting

Experience in the following additional topic areas is preferred, but not required:

  • Cancer screening (colorectal, breast, and cervical)
  • Behavioral health integration
  • Substance use disorder treatment, including opioid abuse
  • Oral health and oral health integration
  • Special populations including:  individuals dealing with housing insecurity or homelessness, and LGBTQ
  • Social Determinants of Health (SDOH)
  • Trauma-Informed Care
  • Quality measure collection, analysis, and reporting
  • Best practices for use of available EHR, HIT, and HIE tools, including Meaningful Use (MU)
  • Value-based payment and population health management

The Health Center Operations Consultant will coordinate key QI activities and projects. The ideal candidate brings a broad background of experience in healthcare or public health with direct experience with primary care settings. This talented professional has the ability to balance multiple priorities and creatively use available resources to drive transformation. Excellent training and meeting-facilitation skills are a must with the ability to build relationships with diverse stakeholders. This professional will possess a customer-service orientation, with enthusiasm for finding solutions and connecting resources in support of health center goals. We are seeking someone that thrives in a fast-paced environment and excels at finding solutions to complicated questions. ESSENTIAL DUTIES AND RESPONSIBILITIES:

Provide Technical Support and Training to Health Centers

  • Plan, coordinate, facilitate, and evaluate learning sessions/trainings for health center staff in collaboration with project partners including curriculum design, agenda setting, and trainer identification.
  • Serve as a resource for members on key content areas – connecting health center staff with the tools and resources they need to reach their care goals.
  • Provide technical assistance to member health centers around identified needs.
  • Work intensively with health center teams participating in assigned projects to refine delivery system and implement best practice concepts.
  • Design, develop, and implement member assessments around clinical performance and TA needs to identify opportunities for improvements and possible solutions.
  • Lead in the coordination of health center Peer Groups, including planning meeting logistics, communicating with members, preparing content for meetings, and directing follow-up coordination.

Knowledge Contribution

  • Work with local and national agencies, independent subject matter experts, and member clinics to identify best practices and evidence-based practices.
  • Conduct research to expand the QI department’s subject matter expertise.
  • Disseminate resources to health center staff and regional bodies in pursuit of project-specific goals.
  • Prepare project reports, memos and maintain project records and statistical information.
  • Support administration of program grants and deliverables including coordination of project activities, tracking deadlines, reporting activities, and convening the stakeholders needed to ensure that projects and products are of the highest quality.
  • Attend relevant stakeholder and advisory meetings to support overall program implementation and evaluation.
  • Other duties as assigned.

Qualifications

Required qualifications and experience:

  • Master’s degree or Bachelor’s degree in relevant field with 5 years of relevant work experience equivalent.
  • Experience working with or in primary care settings/clinics.
  • Knowledge of several subject matters of focus:

-Patient Centered Medical Home (PCMH), team-based care and care coordination

-Chronic disease prevention and management, in particular diabetes and cardiovascular disease

-Quality measure collection, analysis, and reporting

  • Excellent meeting-facilitation skills.
  • Proven ability to teach others and conduct trainings for adult audiences.
  • Excellent written and oral communication skills.
  • Ability to work effectively with diverse teams as well as demonstrated awareness and value of cultural competence and special populations.
  • Proven experience with project management, in particular demonstration of ability to organize various activities and goals, manage multiple projects simultaneously, plan strategically, and see projects through to completion.
  • Strong relationship-building skills, with proven ability to develop effective relationships with diverse stakeholders (government, community, and business environments).
  • Ability to work collaboratively with physicians, staff and external organizations.
  • Team-player, ability to coordinate work collaboratively and resolve conflicts in a precise and timely manner.
  • Effective analytical and problem-solving skills, with the ability to quickly gather information (quantitative and qualitative), identify trends, and propose solutions.
  • High degree of proficiency with MS Office products, including Word, Excel, PowerPoint, and Outlook.

Preferred qualifications:

  • Experience in or with a Federally Qualified Health Center (FQHC)

ADA Specifications

This position will be based in an office in Washington D.C. but will require travel to health centers throughout D.C. This position is primarily sedentary and may require some bending and lifting.
