The Cook Hospital is seeking a Physical Therapist to join our Rehab Department.

The staff Physical Therapist is directly
responsible to the Director and/or his or her designee for direct and
non-direct patient care issues. The
staff Physical Therapist has the responsibility for direction, utilization,
supervision, instruction and evaluation of professional and supportive staff,
volunteers and students in their area of the physical therapy
department/facility.

Licensure/Certification/Registration: Must have, or be eligible for, State Licensure or Registration. Registration must be obtained within oneyear, or as per state law. If not registered, must have temporaryregistration. Must have verification card of current
instruction in CPR.                                          

Education: Graduate of and have a certificate from an American Physical Therapy Association - approved school of Physical Therapy,or a school approved by the World Congress of PhysicalTherapy.

Experience: Preferred minimum of two years experience.
