Connected Home Health & Hospice is seeking an experienced RN to provide after hour/weekend triage for home health and hospice patients in Portland, Oregon and surrounding areas
- Full-time
- Work from home triaging calls
o Friday 5pm – Monday 8am
o Work with RN & LPN team to do visits and take backup call - Compensation based on experience

Job Description
The ideal candidate is an experienced, patient-centered nurse who is home health savvy with excellent triage skills, autonomous, team/culture oriented, driven, creative, fun (!), technologically adept, quality focused, positive, and overall awesome.
Qualifications
- RN, licensed in the state of Oregon, and with at least 1 year home health and hospice experience
- Valid Driver's License in the State of Oregon
- Current vehicle insurance
- CPR certification required
- Strong written and oral communication skills in person and over the phone
- Ability to maintain complete and timely documentation with thorough/efficient reporting to the regular care team
- Experience with Homecare Homebase EHR is preferred
- Strong team skills
- Organized
- Flexible--responds well to changing priorities and conditions
- Knowledge of the duties and functions of the home health and hospice agency and its relationship to the healthcare community
- Comprehensive knowledge of state and federal guidelines and regulations
