Job Description
Community Living Alliance (CLA) is currently seeking individuals who have a passion for helping others to provide personal cares, meal preparation and light housekeeping to multiple clients in accordance with each individual client's plan of care and based on the schedule provided.

Principle Job Duties:
•Provides personal care and housekeeping tasks to clients
•Assists in the mentoring and training of new PCWs
•Transports clients in the employee's vehicle to medical appointments, other appointments and meetings as assigned
•Communicates regularly with the On-Call Coordinator on the status of the clients care and provides appropriate and timely documentation of daily tasks and client cares and needs
•Participates in the rotation for On-Call Scheduler shifts
Qualifications
•Education: High School Diploma or equivalent required
•Experience
◦One year favorable continuous employment with the same employer in the last two years
◦Experience with home care and/or community based case within the last two years is preferred
◦Experience training peers on the provision of providing personal care preferred
•Must be at least 18 years old
•Current Certified Nursing Assistant (CNA) licensed in Wisconsin (other related medical or clinical training may be considered in lieu of CNA)
•Must be able to undergo a criminal background check and meet the requirements for hiring
•Knowledge of:
◦Functionality and understanding of medical equipment and supplies
◦Community-based long-term care and personal cares
•Skills:
◦Excellent customer service skills
◦Excellent communication skills
◦Ability to prioritize and work under pressure
◦Conflict resolution
◦Computer skills (email, internet and Microsoft Office suite) preferred
◦Cell phone and voicemail
◦Flexibility to deal with constantly changing tasks within assigned schedule
•Abilities:
◦Lift 50 pounds without restrictions
◦Complete necessary documentation thoroughly and correctly
•Availability:
◦Must be available to work every other weekend (Saturday and Sunday) as scheduled upon hire
