We are seeking a Chief Population Health and Care Management Officer for our growing organization.
The Chief Population Health and Care Management Officer (CPHO) reports to the President & CEO and is directly responsible for the overall strategic direction and coordination of all population health, care management, and outcome components of value-based contract arrangements. Serves as a principal thought leader for population health program development and research, providing strategic vision and direction to departments to ensure CHN's transition to implementing innovative models of health care delivery, and execution of population health strategies. These strategies are intended to lead to high quality, patient-centric coordinated health outcomes for CHN patients, including streamlining and coordinating complex case management, disease management programs, health risk assessment, wellness and lifestyle management strategies, health education programs, outreach and other programs for specific populations under care management.
While overseeing all care management functions, and in collaboration with Medical Affairs, Legal, Finance, Quality, Data/Informatics as well as health plans develop, the CPHO will implement tools and solutions to monitor performance and outcomes, specifically population health indicators such as patient activation, preventative health risk, social and behavioral health risk, and disease-specific clinical outcomes with the goal of improving patient quality, cost, experience, clinician satisfaction as well as CHN's revenue potential.
This individual also chairs CHN’s Inter-Departmental Population Health Committee to improve collaboration around Value Based Payment (VBP) contracts, setting goals that align with quality metrics and incentive payments, and streamline care management for all populations in need.
The ideal candidate will have a Master's degree, preferably an MHA, MBA, or MPH, and demonstrated experience qualifying them for a senior-level position. Candidates with clinical backgrounds are desirable.
