What We Do
San Francisco is a pioneer in homeless services and a leader in providing supportive housing as a permanent exit from homelessness. The Department of Homelessness and Supportive Housing (HSH), founded in 2016, is a national leader in the movement to end homelessness by developing a coordinated, equity-driven, client-focused system of compassionate services while piloting innovative models, and implementing best practice solutions with measurable results. HSH’s Homelessness Response System (HRS) oversees and implements a system of care that serves 14,000 people daily. Major programs include: street outreach and service connection through the Homeless Outreach Team (SFHOT); 3,000-bed shelter system for adults and families including shelters for members of the LGBT community and survivors of domestic violence; Navigation Centers that provide temporary shelter to chronically homeless individuals using a low-barrier entry model; rapid rehousing rental subsidies for families, adults, seniors and transitional aged youth; the Problem Solving/Diversional programs including the Homeward Bound program which has helped 11,000 individuals return to stable housing in their city of origin; and robust supportive housing programs over 8,000 units which provide permanent housing and services to formerly homeless individuals and families.

Job Description

Overview Job Description

This recruitment will result in a Temporary Provisional appointment. This is not a permanent appointment. A provisional appointee must participate and be successful in a Civil Service examination for this classification and be selected through an open competitive process in order to be considered for permanent appointment. Provisional appointments may not exceed three (3) years.

HSH leads the six core components of the Homeless Response System:
Problem Solving provides opportunities to prevent people from entering the Homelessness Response System and to redirect people who can resolve their homelessness without the need for ongoing support. It may offer a range of one-time assistance, including eviction prevention, legal services, relocation programs (Homeward Bound), family reunification, mediation, move-in assistance, and flexible grants to address issues related to housing and employment.


Coordinated Entry organizes the Homelessness Response System with a common, population specific assessment, a centralized data system and “by name” database of clients, and a prioritization method. This directs clients to the appropriate resources and allows for data-driven decision making and performance-based accountability. The Coordinated Entry process is organized to serve three subpopulations: Adults, Families with Children, and Youth. The process is comprised of four parts: access, assessment, prioritization, and referral.

The ONE System is an enterprise SaaS (Software as a Service)-based system that supports HSH and its 60+ community-based organizations to provide a single unified system for the City’s Homelessness Response System.


Street Outreach connects those living outside with the Homelessness Response System. This includes outreach and engagement (San Francisco Homeless Outreach Team), encampment resolution (Encampment Resolution Team), care coordination, Access Points, and Resource Centers.


Temporary Shelter provides temporary places for people to stay while accessing other services and seeking housing solutions. This may include shelters, Navigation Centers, Stabilization Beds, and Transitional Housing. Housing provides permanent solutions to homelessness through subsidies and housing placements. This may include time-limited supports such as Rapid Rehousing and time-flexible programs such as Rent Subsidies and Permanent Supportive Housing.


Housing Ladder offers opportunities for residents of Permanent Supportive Housing or Rapid Rehousing to move outside of the Homelessness Response System (Moving On Initiative).


Examples of Essential Duties and Responsibilities
1. Conducts outreach and informational presentations at emergency shelters, homeless resource centers, supportive housing sites.
2. Provides other related programs and services to clients who are experiencing homelessness in San Francisco.
3. Applies eligibility criteria and complete intakes.
4. Maintains all required logs to track clients and outcomes.
5. Makes phone calls according to eligibility criteria.
6. Performs other related duties as assigned.

Qualifications

The below qualifications reflect special conditions associated with the position(s) to be filled. They may differ from the standard minimum qualifications associated with this class code.


1. One (1) year of verifiable experience within the last five (5) years, performing a combination of at least two (2) of the following duties: Serving as a liaison between targeted communities and healthcare agencies; providing culturally appropriate health education/information and outreach to targeted populations; providing referral and follow up services or otherwise coordinating care; providing informal counseling, social support and advocacy to targeted populations; escorting and transporting clients; providing courier/dispatcher functions; performing pre-clinical examinations of vital statistics, such as measuring a patient’s weight, height, temperature and blood pressure.
2. Possession of a valid California Class C Driver's License and the ability to pass HSH's SAMBA driver safety check.
3. DMV Record.
4. Ability to lift a minimum of fifty (50) pounds
5. One (1) year of homeless programs experience. Experience with homeless programs include supportive housing, street outreach, temporary shelters, coordinated entry, subsidy administration, or whole person care for clients facing homelessness.
Substitution:
Possession of a Community Health Worker Certificate from City College of San Francisco can substitute for 6 months of experience listed in number one above.
