What We Do
San Francisco is a pioneer in homeless services and a leader in providing supportive housing as a permanent exit from homelessness. The Department of Homelessness and Supportive Housing (HSH), founded in 2016, is a national leader in the movement to end homelessness by developing a coordinated, equity-driven, client-focused system of compassionate services while piloting innovative models, and implementing best practice solutions with measurable results. HSH’s Homelessness Response System (HRS) oversees and implements a system of care that serves 14,000 people daily. Major programs include: street outreach and service connection through the Homeless Outreach Team (SFHOT); 3,000-bed shelter system for adults and families including shelters for members of the LGBT community and survivors of domestic violence; Navigation Centers that provide temporary shelter to chronically homeless individuals using a low-barrier entry model; rapid rehousing rental subsidies for families, adults, seniors and transitional aged youth; the Problem Solving/Diversional programs including the Homeward Bound program which has helped 11,000 individuals return to stable housing in their city of origin; and robust supportive housing programs over 8,000 units which provide permanent housing and services to formerly homeless individuals and families.

Job Description

San Francisco Homeless Outreach Team (SFHOT) is a dynamic and exciting program that works collaboratively in multidisciplinary teams to engage and stabilize chronically homeless individuals and to help establish care for chronic conditions and find permanent housing. The Senior Behavioral Health Clinician provides direct clinical supervision and administrative coordination for Social Work Supervisors and the Housing Coordinator, along with clinical support to the Case Management team.

Under the supervision of the SFHOT Director, the Senior Behavioral Health Clinician provides leadership overseeing client outcomes, i.e., engagement, access to benefits, medical and behavioral health care, and access to transitional and/or permanent housing, while providing quality assurance and operational oversight.

The Senior Behavioral Health Clinician functions within the Leadership Team of SFHOT. The program is comprised of staff from Department of Homelessness and Supportive Housing (HSH), Department of Public Health (DPH) and non-profit contract staff through Heluna Health. The Senior Behavioral Health Clinician’s work occurs within the context of best clinical practices including harm reduction and trauma informed care.

Essential Duties and Responsibilities:

  • Responds to Team requests for support, especially around crisis intervention; provides effective de-escalation and crisis intervention including placement of 5150s, evaluates and stabilizes clients who are homeless, provides home visits in street environments, shelters, SROs, treatment facilities and jail/prison as needed; works frequently in the public eye, occasionally under media scrutiny.
  • Provides weekly individual and group clinical supervision for 2 or more Social Work Supervisors/Social Workers and approximately 20 Case Managers with focus on needs for support, creating a recovery and wellness service environment and resolution of problems in living.
  • Assists with applicant interviews, service recipients, and others concerned for the purpose of securing information to determine eligibility for medical and/or behavioral health care, case management and ancillary services for homeless individuals with SFHOT/HSH utilizing motivational interviewing skills and other appropriate clinical interventions.
  • Utilizes a variety of electronic databases to investigate client status, chart, and review staff performance.
  • Monitors or assists Case Managers or Street Outreach staff in monitoring stabilization rooms and shelter bed resources; maintains effective working relationships with staff members at stabilization room hotels and shelters; directly supervises the Housing Coordinator in their role of coordinating the SFHOT stabilization program.
  • Analyzes cases on team roster to examine current medical and psychosocial vulnerabilities and ensure appropriate implementation of treatment plans. Assists staff and clients with explanation of diagnosis and service plan development, prepares notes for individual cases which may include diagnostic assessment and treatment records, such as therapy notes.
  • Maintains records of all activities relating to client care, prepares periodic reports as needed, confers with supervisors and management team on formulation of policies and procedures; attends case conferences with other clinics and agencies.
  • Establishes positive relationships with clients, staff, and other providers in community; fosters cooperative work environment.
  • Makes presentations regarding SFHOT via public meetings, media, and publications, as directed

Qualifications

Experience:
Two (2) years of verifiable experience as a Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Social Worker (LCSW), or Licensed Professional Clinical Counselor (LPCC).

License and Certification:
Possession of a valid license as a Licensed Clinical Social Worker (LCSW), issued by the California Board of Behavioral Sciences (BBS); OR

Possession of a valid license as a Licensed Marriage and Family Therapist (LMFT) , issued by the California Board of Behavioral Sciences (BBS); OR

Possession of a valid license as a Licensed Professional Clinical Counselor (LPCC) , issued by the California Board of Behavioral Sciences (BBS).

DESIRED QUALIFICATIONS:

  • Possession of a valid California Class C Driver's License
  • Ability to pass HSH's SAMBA driver safety check and maintain insurability
  • Ability to walk a great deal
  • Master’s degree in Social Work or related field with specialization in either psychiatric or medical social work with one year of experience in an inpatient setting, outpatient facility or in direct service with homeless populations
  • Must be able to assist people with disabilities and people with little or no hygiene awareness
  • Must be able to use standard office equipment proficiently
  • Must be able to successfully complete tasks in an environment where interruptions may be constant, background noise is present and may be in the public streets
  • Experience in a community-based setting serving ethnically diverse, low-income, disenfranchised clients
  • Knowledge of community resources and experience in community settings related to homeless populations
  • Analytical ability and skills to handle and solve complex organizational and clinical issues and problems
  • Excellent oral and written communication skills
  • Conflict resolutions skills
  • Customer Service orientation
  • Language capability in Spanish, Cantonese, Mandarin or Vietnamese

Salary Range: $99,840 - $121,316
