Appointment Type

Temporary-Exempt (TEX). This position is excluded by the Charter from the competitive civil service examination process and shall serve at the discretion of the Appointing Officer. The duration of the appointment shall not exceed 24 months.

What We Do

San Francisco is a pioneer in homeless services and a leader in providing supportive housing as a permanent exit from homelessness. The Department of Homelessness and Supportive Housing (HSH), founded in 2016, is a national leader in the movement to end homelessness by developing a coordinated, equity-driven, client-focused system of compassionate services while piloting innovative models, and implementing best practice solutions with measurable results. HSH’s Homelessness Response System (HRS) funds and administers a system of care that shelters, houses, and serves approximately 14,000 people daily. Major programs include: street outreach and service connection through the Homeless Outreach Team (SFHOT); 3,000-bed shelter system for adults and families including shelters for members of the LGBTQ community; rapid rehousing rental subsidies for families, adults, older adults, and transitional aged youth; the Problem Solving and flexible financial assistance programs; and robust supportive housing programs with nearly 10,000 units which provide permanent housing and services to formerly homeless individuals and families.

Job Description

What You’ll Be Doing

Essential Duties and Responsibilities:

  • Provide transport for ambulatory clients accessing HSH temporary shelter, permanent supportive housing sites, or other homeless response system resources.
  • Escort clients into the service site to which they are transported to ensure that they are received for intake by the service provider. Troubleshoot issues between transport dispatch and the receiving service provider as needed.
  • Reports malfunctions of the vehicle to supervisor.
  • Interact in a respectful and professional manner with homeless clients. During transport, answer client questions about homeless response system services.  Provide clients with shelter site information including informing about intake procedures and shelter expectations and rules.
  • Pick up / deliver supplies and equipment as needed.
  • Maintain professional and positive relationships with other service providers, community groups, and the neighborhood.
  • When in the field, interact with HSOC team members to get a sense of the day’s operational and client needs. 
  • Use assigned communication devices (cell phone) to communicate (via voice, text, and email) with team members and other service providers appropriately.
  • Proficient in web-based technologies to look up information and collateral data relevant to service delivery (e.g., Microsoft word, ONE System and Outlook).
  • Document all client transports and conversations in ONE. Maintain timely and accurate documentation according to program requirements.
  • Adhere to professional ethics as well as established policies and procedures.
  • Complete in person and virtual trainings on time and attend all mandatory meetings.
  • Other duties as assigned.

Qualifications

Experience:
One (1) year of verifiable experience within the last five (5) years, performing a combination of at least two (2) of the following duties: Serving as a liaison between targeted communities and healthcare agencies; providing culturally appropriate health education/information and outreach to targeted populations; providing referral and follow up services or otherwise coordinating care; providing informal counseling, social support and advocacy to targeted populations; escorting and transporting clients; providing courier/dispatcher functions; performing pre-clinical examinations of vital statistics, such as measuring a patient’s weight, height, temperature and blood pressure.

License and Certification:
Possession of a valid California Driver's License.

Substitution:
Possession of a Community Health Worker Certificate from City College of San Francisco can substitute for 6 months of experience.

Additional Requirements:

  • 3 months of homeless programs experience. Experience with homeless programs include supportive housing, street outreach, temporary shelters, coordinated entry, subsidy administration, or whole person care for clients facing homelessness.
  • DMV record required upon referral.
  • Insurable by Department’s Auto Policy.
  • Ability to lift objects weighing up to fifty (50) pounds.
