Job Description
We are looking for: 1 Full-time Contract Case Manager, Holocaust Survivor Program
Salary: Commensurate with Experience
Reporting to the Manager of Client Services, the Case Manager will manage a caseload of Holocaust Survivors with complex healthcare and psychosocial needs. This will involve working as part of a dedicated team as well as with staff from across the agency. The role includes work both in the office as well as in the community. Primary functions include client assessment, case management, documentation and ongoing client support.
Responsibilities:
• Support clients to apply to the agency’s Holocaust Survivor Services Program
• Review applications in accordance with funder guidelines
• Complete in-home client assessment and create corresponding care plans
• Complete documentation and maintain client files in accordance with agency protocols
• Provide support and case management to clients by phone and in the community
• Assist client to apply for resources and services through community and partner agencies
• Plan and implement new supportive programs for clients including volunteer phone calls and the health and wellness program
• Assist with staff education and training programs
Qualifications
• Completion of a Bachelor’s Degree in Social Work, Social Service Work, Registered Nurse, Occupational Therapist or any registered allied healthcare professional and a current member in good standing with the appropriate governing college
• Minimum of 1-3 years of clinical experience is required. Clinical experience in community care, home care, or case management experience is preferred
• Experience working with Holocaust Survivors and or/ Survivors of trauma
• A strong knowledge of community health providers and home care options and other local services is required
• Must be able to work well independently and within a dynamic team
• Demonstrates skilled ability to work with a variety of clients across the spectrum
• Excellent organizational, communication (verbal and written), computer literacy, and interpersonal skills
• Strong advocacy skills and a patient-centered focus, strong decision making, problem recognition, problem solving skills, negotiation and conflict resolution skills
• Ability to perform routine work independently
• Computer proficiency including a thorough knowledge of MS Word and Excel computer software
• Russian language skills are required
• Must be willing to travel. This position will require a valid driver’s license and vehicle
