Job Description

Position Summary

Join our team as a QMAP Personal Care Partner II and make a real difference in residents' lives! You'll be the go-to person for meeting daily personal care needs while embodying our company's mission and values. Think of yourself as a care detective—anticipating needs, identifying concerns, and keeping our licensed nursing team in the loop. Plus, you'll play a key role in medication supervision, ensuring every resident gets the right medication, at the right time, in the right way.

What You'll Be Doing (The Fun Stuff!)

 

  • Be a personal care superhero! Respond promptly to residents' needs and lend a hand with daily activities like bathing, dressing, meals, and laundry—all while paying attention to the little details that matter (combed hair, tied shoes, fresh breath!).
  • Channel your inner detective by spotting and reporting care needs before they become big issues—whether it's hygiene concerns, broken glasses, skin changes, or missing hearing aids.
  • Keep the communication flowing! Stay connected with your supervisor and teammates to stay on top of resident needs and celebrate wins together.
  • Be a safety champion by monitoring resident well-being and creating a secure, comfortable environment.
  • Speak up! Report any concerns about abuse, neglect, or misuse of resources—you're an important part of our safeguarding team.
  • Stay alert to changes in resident health or status and keep your supervisor informed every step of the way.
  • Document resident conditions as directed, keeping accurate records that help the whole care team succeed.
  • Support residents with their daily medication routine in compliance with state regulations, and flag any questions or concerns with our RN/LPN team.
  • Keep medication records organized and up-to-date, including documenting any medication refusals or treatment changes.
  • Keep the medication station stocked and ready to go by restocking carts, drawers, and supply areas with essentials like gloves and sanitizer.
  • Spot maintenance needs? Submit work orders to keep our facility running smoothly.
  • Show up and speak up at monthly staff meetings—your insights on resident and departmental needs are valued!
  • Embrace flexibility! Take on additional duties as needed to support our team and residents.

 

Qualifications

Basic Qualifications & Experience

  • Per the state of operation requirements, Medication certification; QMAP-A/L, Med Tech, Med Aide, Nurse Delegation
  • Must be able to read, write and speak the English language.
  • High school or Equivalent (GED)
  • One (1) year experience in some level of elder care preferred
  • Proficient computer skills preferred
  • Strong self-management and organization skills
  • Strong multi-tasking abilities with the ability to handle competing deadlines, flexible and adaptable (Open to change).
  • Ability to effectively handle difficult and sensitive issues and maintain confidentiality

Working Conditions / Physical Requirements

  • Moderate physical activity required by handling objects up to 50 pounds occasionally and/or up to 20 pounds frequently.
  • Constant standing, walking, and grasping. Frequent stooping and reaching above shoulder heights, occasional sitting, crawling, kneeling, and squatting.
  • May have exposure to blood borne pathogens, body excretions and communicable diseases possible.
  • Possible exposure to household chemicals.
  • Sits, stands, bends, and moves intermittently during working hours.
  • May be subject to frequent interruptions.
  • Interacts with clients, family members and visitors frequently.
  •  6pm-6:30am $20.75 - $22.88 / hour
