Job Description

Position Summary

Manages the residents care experience through the community’s onsite Medical Clinic.  Ensures a proper communication flow between care provider, therapies, ancillary services, community team, and their care team.  

Essential Duties

  • Manages the Medical Clinics schedule; conducts appointment reminders, ensures appointments are scheduled per practitioner’s directives, and may assists residents in arriving for the appointment and throughout the visit.
  • Relays messages and escalates appropriate calls to practitioners.
  • Prior to resident appointment, ensures insurance and any updates necessary for a successful appointment are properly recorded.
  • Reviews insurance payments, and follows up with residents on any outstanding balances.
  • Maintains all reports, releases, labs, and miscellaneous information is recorded and readily available in resident’s medical record.
  • Routinely audits charts, ensures charts are accessible to appropriate parties per HIPPA.
  • Takes vitals and correctly documents these at the start of each appointment; weight, blood pressure, temperature, pulse. 
  • Maintains clinics cleanliness and necessary sterilization and keeps exam rooms readily stocked.
  • Coordinates transportation for off-site appointments.
  • Communicates any follow up needs with appropriate care teams, escalates to appropriate parties if there are complex needs.
  • In supporting the practitioner, may be asked to renew prescriptions and take labs, if properly credentialed.
  • Assists with assessments and care plan development.
  • Assists with wellness assessments and wellness programing.
  • Other duties as assigned.

Qualifications

Basic Qualifications & Experience

  • Certified or Registered Medical Assistant (RMA/CMA) in good standing, preferred
  • C.N.A in good standing, preferred.
  • Medication Administration certification, preferred
  • Holds a current C.P.R. and First Aid course completion certificate.
  • Familiar in working in electronic health medical record system.
  • Knowledgeable with Office suite, Outlook, Word and Excel a must.
  • Knowledgeable with a multi-phone line system.
  • Excellent customer service skills, triaging issues, resolving concerns and following through with promised deliverables. 
  • 3-6 months experience in the senior services industry preferred.
  • Must be able to read, write and speak the English language.

Working Conditions / Physical Requirements

  • Position requires medium-heavy physical work.  Heavy lifting, pushing, pulling objects up to 100 pounds of force occasionally, and/or up in excess of 45 pounds of force repeatedly to move objects. 
  • Stands and walks constantly during working hours with occasional sitting, kneeling, squatting, reaching above should height and fine motor control with fingers.  May also require grasping.
  • Exposure to blood borne pathogens, body excretions and communicable diseases.
  • Is subject to frequent interruptions.
  • Interacts with residents, family members and visitors frequently.
