Care Planner Inc. in Roseville is a family owned Personal Care Agency that has been proudly serving seniors and people with disabilities since 1994. We are a respected and trusted name in the industry and we are seeking a talented and compassionate full-time RN. Our agency works with a variety of age groups, locations, and programs in the Twin Cities metro area.

The position we are looking to fill is a full-time RN for a Monday-Friday schedule with some flexibility. This candidate must have a current MN nursing license (RN) and a passion for working with seniors and people with disabilities and promoting their independence in the community. This position requires travel to client’s homes and the ability to work with a diverse population. Detailed documentation and appropriate and accurate billing is an essential function of this position.

- Excellent time management skills

- Must be a motivational self-starter

- Ability to coordinate and problem solve as part of a team as well as independently

- Knowledge of home care and basic and comprehensive licenses

- Relate professionally with clients, staff, and family members in a caring manner

- Create and implement care plans consistently with state requirement
