Position Summary

The CalAIM Community Supports Program Leader is responsible for overseeing and expanding Care Partners At Home’s CalAIM Community Supports home care program, including personal care and caregiver respite. This role ensures operational excellence, compliance, accurate reporting, payer partnerships, and high-quality service delivery aligned with CalOptima and other Managed Care Organizations (MCOs).

CalAIM Community Supports Program Manager - Requirements: 

  • 3+ years of experience in healthcare program management, home care, or related field.
  • Familiarity with CalAIM or Medi-Cal programs.
  • Strong understanding of home care workflows.
  • Excellent communication and relationship-building skills.
  • Proficiency with Microsoft Office; Excel reporting capabilities.

Key Responsibilities:

Program Leadership & Operations

  • Oversee daily operations of the CalAIM Community Supports program.
  • Manage intake, authorizations, scheduling, and service delivery.
  • Develop and maintain SOPs, workflows, and quality initiatives.
  • Track program performance, productivity, and growth metrics.

Payer & Partner Relations

  • Act as liaison with MCOs (CalOptima, Molina, Health Net, IEHP).
  • Maintain relationships with health plans and community partners.
  • Ensure compliance with payer standards and support audits and reporting.

Compliance & Documentation

  • Ensure adherence to CalAIM regulations and clinical standards.
  • Maintain accurate program reporting and documentation.
  • Implement updates based on DHCS and MCO policy changes.

Data & Performance Oversight

  • Lead operational reporting on hours, utilization, and retention.
  • Monitor progress toward performance and growth goals.
