As a Case Management Assistant (CMA) in our Enhanced Care Management (ECM) program, you’ll be the friendly voice and helping hand connecting our patients to the services and support they deserve.

Case Management Assistant Requirements:

EDUCATION AND EXPERIENCE

  • 1-2 years’ experience in case management, social services, or healthcare coordination.
  • Strong knowledge of local healthcare, community, and social service resources (or a quick learner!)
  • Bilingual in Spanish or Vietnamese - Tech-savvy with solid computer skills (EHR experience is a plus)
  • Valid CA driver’s license & insurance
  • Physical Requirements: - Comfortable sitting, standing, walking, typing, and occasionally lifting up to 25 lbs.

Desired:

  • Knack for building relationships and earning trust.
  • Experience with community outreach or field-based resource linkagel.
  • A heart for serving diverse communities and meeting people where they are.

Essential Functions:

  • Be the first friendly voice for prospective ECM members, inviting them to join our program.
  • Support clients and their families by scheduling, coordinating, and keeping everyone on the same page.
  • Work with our Lead Care Managers (LCMs) to educate clients on all the ways we can support them.
  • Help clients access important community resources and benefits (like SSI, CalFresh, or Community Supports through CalAIM).
  • Be part of a collaborative care team, sharing updates and brainstorming solutions to meet patient needs. - Make periodic check-ins with clients to cheer them on and help them reach their health goals.
  • Play a key role in helping patients transition smoothly from hospital or institutional settings back into their communities.
