Physical Therapist Benefits:

  • Compensation:$120,000 - $140,000
  • Type: Full-Time
  • Location: Tuolumne, CA

This team works with you and your doctor to develop and manage a personalized home health plan that is the most convenient.

Physical Therapist Requirements:

EDUCATION AND EXPERIENCE

  • Possesses a doctorate or grandfathered masters degree in physical therapy approved by an accredited organization.
  • Licensed to practice as physical therapist within the state.
  • Experience as a physical therapist - Community/hospice experience is preferred.
  • Possesses and maintains current CPR certification.
  • Understands hospice philosophy, focus of care is comfort vs. rehab and to maintain care in their residence as long as possible.
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Essential Functions:

Provide physical therapy services tailored to individual patient plans, including assessments, evaluations, and direct treatment to maintain comfort and improve mobility. Responsibilities include:

  • Developing comprehensive care plans and evaluating home environments.
  • Managing patient mobility, strength, gait, range of motion, and transfer skills.
  • Supplying and instructing on the use of therapeutic appliances for increased independence.
  • Educating patients and caregivers about physical therapy goals and appliance care.
  • Leading other staff or family members in certain aspects of patient physical therapy.
  • Managing patient education on prosthetic devices.
  • Documenting clinical progress and participating in discharge planning.
  • Contributing to quality improvement efforts and team orientations.
  • Supervising physical therapy assistants and hospice aides as needed
