More about your responsibilities:

You will be working with your teams to address the patient’s transitions between the hospital and Home Care, Long Term Acute Care and Skilled Nursing Facilities.

The qualified candidate should have three years clinical experience in case management. Two-Five years leadership experience with focus on case and disease management in an acute care hospital setting.

EDUCATION Requirement: Bachelor’s Degree from an accredited school, college or university as a Registered Nurse or Social Worker. Master’s degree is a plus.

MINIMUM EXPERIENCE: 2 years in acute hospital case management and leadership position

Our client uses McKesson InterQual® . Business Planning experience required.

It is preferred you hold Certification in Case Management
