Job Description

Acentra seeks a Social Worker Case Manager (Hybrid - Licensed in Arizona) to join our growing team.

Job Summary:

The Social Worker Case Manager is responsible for:

  • Providing in-home, in-person care coordination in defined areas and telephonic care coordination across the United States.
  • Assessing, planning, implementing, monitoring, and evaluating options and services to affect an appropriate, individualized plan for the beneficiary across the continuum of care.
  • Using independent judgment, utilizing clinical knowledge and competence, communication skills, problem-solving and conflict resolution to effectively ensure optimal client outcomes with consideration to payer requirements.
  • Coordinating beneficiary needs related to community resources, financial assistance programs, and long-term activity of daily living needs outside the benefit criteria of the medical insurance coverage.
  • Working effectively with all healthcare team members internally and externally.

Phone and In-Home Visits 

** This is a full-time, direct hire, exempt (salary), Hybrid opportunity (within specific counties within the State of Arizona) with Benefits. **

** The selected candidate must reside and work in the state of Arizona, within the United States. **

** A clean Motor Vehicle Report (MVR) in the state of Arizona and access to reliable transportation for this role is required for this role. **

** This position is Hybrid with in-home, in-person care coordination (up to approximately 25%) and telephonic care (up to approximately 75%) for (the largest population of) beneficiaries within the NE portion of AZ (Zip Code 86033) **

** Drive time distance of up to a two-hour radius maximum (with mileage reimbursement). **

** The selected candidate is required to have a designated private workspace at their home residence for this role. **

** The first dose of the COVID-19 vaccination is required as you will be working in person with beneficiaries. **

 Job Responsibilities:

  • Use of independent clinical judgment and discretion within the scope of practice to assess, plan, implement, monitor, and evaluate options and services to affect an appropriate, individualized plan with the beneficiary across the continuum of care; utilizes clinical knowledge and competence communication skills, problem-solving and conflict resolution to effectively ensure optimal beneficiary outcomes with consideration to client requirements.
  • Develops beneficiary-centered care plans demonstrating shared accountability between beneficiaries, caregivers, and providers by working collaboratively with the beneficiary, their provider, and other members of the Funds Health Care team as necessary to coordinate care, address identified gaps in care or compliance with the goal to optimize beneficiary function, or prevent further decline in health, consistent with the Funds and Acentra Health “caring” philosophy.
  • Maintains strict standards for client confidentiality and client-related information. Complies with all organizational, state, and federal regulations and policies on confidentiality.
  • Manages caseload to ensure all required documentation and entry of assessment results into the web-based database are completed accurately and timely in accordance with regulatory requirements and company policies and procedures.
  • Maintains open communication with all appropriate parties.
  • Pursues ongoing education, certification, and self-development to remain current with case management standards.

Qualifications

Required Qualifications/Experience:

  • Bachelor's degree in Social Work.
  • Active, unrestricted Social Work license in the state of Arizona
  • 3+ years of Social Work experience with an older adult population in a healthcare environment.
  • MS Office (e.g., Word, Excel, Outlook, Teams)

Preferred Qualifications/Experience

  • Master’s degree in Social Work.
  • Case Management Certificate (CCM).

Required Knowledge, Skills, and Abilities

  • Ability to prioritize with variable and sometimes conflicting deadlines; superior attention to detail and demonstrated decision-making ability.
  • Effective communication (written/verbal), interpersonal, organizational, time management, and communication skills with a strong focus on customer service, including building and maintaining relationships with internal/external customers and facilitating meetings.
  • Ability to work independently and as part of a team.

Compensation

The pay for this position is $70,000/yr

“Based on our compensation philosophy, an applicant’s placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.”
