CASES is looking for fulltime and part-time Psychiatric Nurse Practitioners for the Brooklyn & Manhattan Assertive Community Treatment (ACT) teams. The ACT teams provide mobile community based mental health treatment. The Psychiatric Nurse Practitioner is an integral part of a multi-disciplinary team that includes a psychiatrist, nurses, social workers and peer specialist staff. The team provides 80% of its treatment services in community locations. The Psychiatric Nurse Practitioner will demonstrate a commitment to the assessment and treatment of individuals with serious mental illness and co-occurring substance use disorders.

• Work with the ACT team psychiatrist to deliver psychiatric care
• Provide psychiatric assessments, mental status examinations and pharmacological evaluation services in the community
• Ensure consumers are seen at least monthly and in accordance with ACT licensing standards for the delivery of psychiatric care in the community
• Work collaboratively with the RN and LPN ensuring consumers receive routine lab tests, timely access to medications and effective medication management
• Prescribe medications, teach consumers about side effects and monitor response to prescribed medications
• Administer injectable medications such as Risperdal Consta, Invega Sustenna, Proxilin and Haldol
• Complete medication authorization documentation and communicate with Medicaid Managed Care Organizations (MCOs) to request medication authorization
• Work in collaboration with consumers and significant others to support recovery and rehabilitation goals
• Work collaboratively with the team ensuring consumers receive services based on their immediate needs and current status in ACT treatment
• Provide crisis intervention services working with the team leader to address urgent needs
• Participate in team morning organizational meetings and weekly clinical meetings
• Communicate with hospital staff when consumers access emergency and inpatient services
• Ensure services delivery is flexible, recovery-oriented and trauma-informed
• Provide culturally competent services in accordance with CASES policies
• Effectively use the electronic health record (EHR) AWARDS Foothold Technology
• Ensure documentation is completed within 24-hours of the service contact and are available to be read in the daily organizational meeting
• Promote the integration of primary care working collaboratively with the Manhattan onsite Federally Qualified Health Center (FQHC) medical providers and the community primary care providers serving the consumers ensuring medical needs are addressed and monitored
• Promote and deliver evidence-based integrated treatment for consumers with co-occurring substance use disorders
• Complete buprenorphine training and obtain the waiver to prescribe
• Consistently work in a positive and cooperative manner with fellow team members
• Exercise sound judgment and hold protected health information and other sensitive information in the strictest of confidence in accordance with agency, state, federal 42 CFR and HIPAA confidentiality policies
• Other duties as assigned

•    Valid New York state license for practice as a Psychiatric Nurse Practitioner; 
•    Extensive experience working with people with serious mental illness, and co-occurring substance use disorders, and primary health care problems
•    Experience working with the justice-involved and homeless population with mental illness and knowledgeable about risk assessment and risk management
•    Excellent written and communication skills
•    Bilingual Spanish speaking preferred
