Overview: The Psychiatrist with the team leader has overall clinical responsibility for the delivery of clinical services on the FACT & ACT teams. The FACT & ACT teams provide mobile community based mental health treatment to promote the recovery and community integration of individuals with serious mental illnesses and high rates of co-occurring substance use disorders. The FACT teams serve consumers returning to the community from jails and prisons, participating in alternative-to-incarceration and treatment courts programs, those with histories of high-risk and violent behavior, and individuals on probation and parole. The ACT teams primarily serve consumers with high utilization of psychiatric emergency rooms and inpatient hospital stays. The teams provide 80% of the treatment services in community locations. The Psychiatrist provides assessment and treatment; clinical supervision, education, training of the team; and development, maintenance, and supervision of medication administration and coordination of medical treatment. 

Responsibilities:      

• Conduct initial comprehensive psychiatric evaluations in the community
• Ensure at least one monthly face to face contact with each consumer to review mental status, medication side effects and overall clinical status and increase the frequency of treatment when needed
• Conduct visits in the community in accordance with NYS Office of Mental Health (OMH) licensing standards ensuring consumers are seen at least once each quarter in the community and the first visit with the consumer after admission is conducted in the community
• Coordinate the delivery of the psychiatric care when sharing the caseload with a psychiatric nurse practitioner
• Respond to crisis situations
• Prescribe medications (utilizing e-prescribing function of EHR), teach consumers about side effects and monitor consumer response to prescribed medications
• Coordinate consumer access to medications with the RN and LPN nurses including administering injectable medications when needed
• Order routine blood work and/or toxicology testing as needed
• Promote and deliver evidence-based integrated treatment for consumers with co-occurring substance use disorders
• Complete buprenorphine training and obtain the waiver to prescribe
• Promote the integration of primary care working collaboratively with the Manhattan onsite Federally Qualified Health Center (FQHC) medical providers and the community primary care providers serving the consumers on the team ensuring medical needs are addressed and monitored
• Provide psycho-education to family members and significant others
• Complete assessments, progress notes, and significant data such as incident reports, utilizing the AWARDS Foothold Technology EHR
• Ensure progress notes are completed within 24-hours of the delivery of the service and are available to be read in the daily organizational meeting
• Use motivational interviewing, recovery and trauma-informed approaches when delivering services
• Coordinate care with family and other community treatment providers or collaterals including, but not limited to: hospitals, housing providers, shelter staff, criminal justice personnel, AOT, and primary health care providers
• Triage emergency contacts from program clinicians and provide guidance and input
• Provide services to consumers based on current risk and status
• Provide trainings and clinical supervision to staff
• Participate in morning organizational meetings and staff meetings
• Review initial service plans and 6-month service plan reviews and signs documents to coordinate and authorize the treatment plan;
• Complete medication authorization documentation and communicate with Medicaid Managed Care Organizations (MCOs) to request medication authorization
• Communicate with the Department of Health and Mental Hygiene Assisted Outpatient Treatment (AOT) team responding to requests for information about consumers with AOT orders
• Consistently work in a positive and cooperative manner with fellow team members
• Exercise sound judgment and hold protected health information and other sensitive information in the strictest of confidence in accordance with agency, state, federal 42 CFR and HIPAA confidentiality policies
• Other duties as assigned

• Psychiatrists licensed as a physician by the NYS Education Department;
• Psychiatrists certified by, or be eligible to be certified by, the American Board of Psychiatry and Neurology;
• Extensive experience working with people with serious mental illness and co-occurring substance abuse and primary health care problems;
• Experience working with the justice-involved population with serious mental illness; and
• Excellent written and communication skills, and
• Bilingual Spanish speaking preferred.
