CASES is looking for a fulltime Psychiatric Nurse Practitioner for our mobile treatment and outpatient mental health clinic. The Psychiatric Nurse Practitioner will demonstrate a commitment to the assessment and treatment of individuals involved in the criminal justice system. The Psychiatric Nurse Practitioner will mainly work in an outpatient mental health clinic located in Central Harlem and will also provide some coverage as part of a mobile mental health treatment team. The psychiatrist nurse practitioner will complete initial psychiatric evaluation, prescribing and monitoring medications and coordination of care with therapists, integrated primary care staff, and Medical Director. Working collaboratively with therapists ensures individual and group psychotherapy services are care coordination is delivered within the framework of client self-determination, recovery, and cultural competency. Creativity and flexibility is required to support the successful deliver of services to a high need population/

 

Job Description

Conduct initial psychiatric evaluation

Conduct mental status examinations and pharmacological evaluations

 Prescribe medications, teach clients about side effects and monitor response to prescribed medications

Review and sign comprehensive assessments completed by therapists

Provide recommendations for treatment plans, review and sign treatment plans and treatment plan reviews

Administer injectable medications including (Risperdal Consta, Sustenna Invega, Proxilin and Haldol)

 Provide services to clients based on current status, risk, and as clinically indicated

 Provide clinical supervision to staff on the type and nature of ongoing psychotherapy needs (individual and  group)

Support staff to provide recovery-oriented and trauma-informed services

 Participate in staff meetings

Liaison with AOT to establish request for AOT exam in select cases

 Provide crisis intervention services supporting staff as needed

 Communicate with hospital ER attending and residents when clients access emergency services

 Support discharge planning with hospital and ER staff

Conduct risk assessment, provide guidance and direction on risk management

Use e-prescribing

Complete progress notes, significant events, referrals, and evaluations within expected timeframes

 Use Electronic Medical Record (EMR) and data reports from EMR to track treatment progress and individual client outcomes

Maintains at least an 75% show rate for clients enrolled in clinic and scheduled for regular appointments

Provide crisis intervention services

Use collaborative documentation procedures for assessment and individual sessions with clients

Communicate with primary healthcare providers and deliver integrated treatment in coordination with primary care behavioral health integrated wellness services and co-located medical services

Work with onsite family nurse practitioner providing healthcare and health monitoring to ensure integration of primary care

Work in collaboration with the client and significant others to support recovery goals

Participate in Incident Review Committee (IRC) quarterly meetings

Provide culturally competent services in accordance with CASES policies

Participate in quality assurance, training, and supervision activities established by the supervising consultant psychiatrist and senior staff

Any other duties as required by senior CASES staff

Qualifications

Valid New York state license for practice as a Psychiatric Nurse Practitioner;

Extensive experience working with people with serious mental illness, and co-occurring substance use disorders, and primary health care problems

Experience working with the justice-involved and homeless population with mental illness and knowledgeable about risk assessment and risk management

 Excellent written and communication skills

 Bilingual Spanish speaking preferred
