The Behavioral Health division consists of a continuum of services for justice-involved individuals with serious mental illness, including outpatient clinic, health home care management (HHCM), assertive community treatment (ACT), and intensive mobile treatment (IMT). The Care Manager is responsible for providing health home care management services to individuals with serious mental illness, substance use disorders, and criminal justice involvement – with a focus on the Health Home Plus (HH+) population.

Job Description

•    Provide core care management services, including Comprehensive Care Management, Care Coordination and Health Promotion, Comprehensive Transitional Care, Enrollee and Family Support, Referral to Community and Social Supports, Use of Health Information Technology to Link Services.
•    Plan and coordinate care management needs, including referral and escort to medical, psychiatric, substance use, crisis intervention, legal, housing, education/employment, entitlements, social, and peer support services. 
•    Maintain engagement with individuals who are often disengaged from care, have difficulty adhering to treatment recommendations, or have a history of homelessness, or criminal justice involvement.
•    Conduct screening and assessment of high-risk needs, such as substance use, psychiatric symptoms, harm to self/others, abuse/neglect, and domestic violence.
•    Assess clients’ eligibility for Home and Community Based Services (HCBS) and work with Managed Care Organizations (MCOs) and community HCBS providers to make referrals as indicated.
•    Utilize electronic databases as needed to perform work functions (e.g., ePACES/eMedNY, Health Commerce System Medicaid Analytics Performance Portal, GSI Health Coordinator)
•    Complete all documentation within required time frames
•    Provide care management services in a strengths-based, recovery-oriented, trauma-informed, and culturally sensitive manner
•    Abide by all policies and procedures set forth by the NY Department of Health, partner Health Homes, and the CASES Care Management program. Keep abreast of city, state, and federal regulations, laws, and initiatives pertaining to behavioral health Medicaid managed care benefits. Attend relevant trainings.
•    Other duties as requested by the Team Leader or CASES senior management

Qualifications

  • Bilingual Spanish-speaking required
  • Bachelor’s Degree with a major or concentration in social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing or other human services field.
  • Two years of experience in (a) providing direct services to people with serious mental illness, developmental disabilities, or alcoholism or substance abuse or in (b) linking individuals with serious mental Illness, developmental disabilities, or alcoholism or substance abuse to a broad range of services essential to successful living in a community setting.
  • Advanced clinical skills in the engagement of high-need individuals with serious mental illness, including motivational interviewing, suicide prevention, risk screening, trauma-informed care, person-centered care planning and interventions, and recovery-oriented approaches. 
  • Strong commitment to serving justice-involved individuals with behavioral health needs.
  • Bilingual Spanish-speaking preferred.
