The concept developed in the Netherlands, professional nurses work in autonomous teams (up to 12 nurses) and are responsible for all care needs of their clients, both professional and personal. Nurse professionals are capable of self-management and effective collaboration, therefore middle management is not necessary. With the focus on the relationship as the starting point of care and belief in the intrinsic value of community care, bringing highly skilled nurses to our neighborhoods results in delivery of higher quality care, better outcomes, and ultimately lower costs.
