This is a Full-Time (Exempt) position : 40 hrs. (8:00 a.m. - 5:00 p.m.) Monday through Friday & Week-Ends as needed.
Under the direction of the Health Services Administrator, and in collaboration with the Interdisciplinary Team at Broadmead, the Health Services Admissions Director is responsible for the successful pre-admission and admission experience to include the transition from hospital/acute care/home to sub-acute rehabilitation; coordination of skilled rehabilitation services, and oversight of the short-stay census at Broadmead.
The Health Services Admissions Director will maintain the annual budgeted short-stay occupancy census while ensuring a successful admission and patient/family/representative experience.
Ability to establish rapport with hospital case managers, physician groups and home health providers, by frequently visiting referring hospitals and offering in-service presentations.
Able to respond to referrals with a prompt one-hour turnaround time.
Able to analyze lab values, disease process, and discharge planning goals.
Work closely with various interdisciplinary team members
Hospital and home visits to verify admission appropriateness
Broadmead Welcome Packet and admission contract review with patient and family/representative.
Post admission follow up the next business day and an effective hand-off to the Interdisciplinary Team.
Daily interaction with the Hospital Liaison concerning bed-availability and potential patient status.
Complete patient admission criteria evaluations.
Participate in interdisciplinary care plan meetings, as needed.
Secure approval of insurance and financial information prior to admission to include managed care authorization numbers.
Provide in-services as needed to staff.
Serve on facility wide committees as requested.
Be aware of and comply with HIPAA security regulations.
Help ensure a positive short-stay admission experience by monitoring and verifying a variety of key procedures including, but not limited to:
Room readiness.(competing the room readiness checklist)
Pre-admission checklist and compliance with the admission criteria policy.
Nursing department communication and readiness.
Family interactions prior to and after admission
Adjustment to understanding/acceptance of medical diagnosis, treatment or procedures.
Accountable for promoting internal customer service.
Manage the resident satisfaction survey upon admission and upon discharge. Maintain data and share with QAPI Committee.
Maintain a monthly database of admissions, discharges, declinations and hospital readmissions to help drive performance and adjustments to process.
Abide by Broadmeads Standards of Community Excellence.
Possess a valid Maryland driver’s license and vehicle
Perform other related duties as required
3 - 5 experience in the field of aging, preferred.
Comprehensive knowledge of the normal aging process.
General knowledge of the State and Federal laws and regulations concerning the provision of health care to patients in skilled nursing facilities to include sub-acute rehabilitation.
General knowledge of available resources for older adults.
Bachelors Degree of Business, Nursing, Social Work or similar Health Services Program preferred.
Marketing or clinical background helpful.
Has latitude to deal with most problems.
Refers unusual problems to supervisor.
Ability to work at multiple tasks at the same time.
Able to manage utilization review to include managing discharge dates to foresee available admission options.
Ensure high quality of care to patients and Broadmead residents through development and implementation of effective pre-admission and admission programs.
Short-stay census according to annual budgeted occupancy.
Positive hospital and provider relationships to support referrals for sub-acute rehabilitation.
Create and maintain community relationships with networking organizations (such as hospitals, physician offices and provider referral groups).
